I am not able to do so at all. Though that is possible within private messages.

Could the forum administrators please enable the [IMG] code for all members of this forum?
Why is it being turned off? Let's not bother about davieddy or cmd or whoever posts
with obscene images over posts within this forum. If someone does like that, then
just simply immediately delete off with that post and then ban that user.

If you quote his post, you can see that his code was [ ATTACH ]5139[ /ATTACH ] (without the spaces, I had to add those to let you see it here). You can attach an image and then put it inline with that code. (if you're using the Enhanced Interface you can do this easily with the Attachments button (looks like a paperclip).
Like this:

I don't think you can. I can't seem to add attachments from other posts to mine. It just looks like:

The exception would be if you add more than one attachment to a post. This is only possible, given the current settings, by being a moderator in the forum you're in, posting two (or three or ...) times with one attachment each, then merging the posts.
