In Azerbaijan it is usually accompanied by saying “Stones at your head” which is a common insult.
ETA: BTW, in Azerbaijan just north of Iran, a Thumbs-up is a Thumbs-up.
ETA II: In Germany 31 reverse side in front of the face means nuts, which is the same as 2 pointing at ones head in the rest of the world.
ETA III: 17 is a common gesture by youngsters in selfies. Have no clue what it means.

17 is also a horns symbol, so signifying either the devil or cuckoldry.

I know 17 as a gesture in relation to surfers, mainly from the movies.
