I believe that 189 refers to the number of exponents below M(57885161) still to be tested the first time. I am sure someone will correct this if I am mistaken.

That is based upon the computers that those exponents are assigned to. We still have some old computers with some old assignments. They are still making slow progress. They are being watched. If they fail to meet minimum standards, the exponents can be reassigned.

Related question of my own: are exponents with one suspect LL test (bad error code) counted as having been tested once for the purposes of this countdown milestone?

I don't know but this kind of thinking is part of my reasoning for prioritizing the double- or triple- checking of suspect results (and perhaps the results of known unreliable sources).
